Who Is Actually Working as an ADHD Coach
The field was counted for the first time in 2026. Margaret Sibley of the University of Washington School of Medicine and colleagues surveyed 481 US ADHD coaches and published the results in JAMA Network Open. The numbers describe the market you are shopping in:
85% hold no professional medical licence.
About 9 in 10 had no professional mental health work before coaching.
90.5% have no formal clinical supervision.
60.9% started during or after the COVID-19 pandemic.
72.7% have ADHD themselves or suspect they do.
Just over 60% completed a training curriculum endorsed by the ADHD Coaches Organization.
78.9% refer clients out for formal ADHD evaluation.
53% addressed substance use or addiction inside coaching sessions.
48.7% worked with trauma.
41.6% addressed suicidality, abuse, or harm to self or others.
Those last 3 figures are the reason to ask hard questions of everyone in this market, us included. An unlicensed, unsupervised practitioner working on suicidality is outside what coaching is for.
None of this makes coaching a bad purchase. A coach with ADHD who trained after 2020 can be the right hire, and lived experience is a real asset in this work. It does mean the credential stack does less work than the marketing implies, so here it is accurately:
PAAC issues ADHD-specific credentials. Its published requirements are 60 ADHD coaching hours for the CACP, 90% of which must be paid, and 250 hours for the PCAC on the same paid basis.
ADDCA and similar academies deliver training that can feed those credentials.
ACO maintains a directory with its own listing requirements.
ICF credentials (ACC, PCC, MCC) are general coaching credentials. They are not ADHD-specific.
A California licence is a different product entirely. See the middle column.

Do you bill insurance, provide a superbill, or stay cash only?
Is this in person or by video, and which one am I booking?
Can you write an accommodation letter a school or employer will act on? If not, who do you send people to?
How will we know at week 6 whether this is working? What are you measuring?
Who is this not for?
Walk away from a guaranteed outcome, from anyone who cannot explain the line between coaching and therapy in 1 sentence, and from anyone who will not answer question 4 before a consultation.
What the Evidence Says as of March 2026
ADHD coaching has promising evidence and a thin evidence base. Both are true, and pages in this category usually publish only the first half.
The strongest recent adult result: Elizabeth Ahmann, Micah Saviet and Mark Otto published "Coaching for Adults With ADHD: A Prospective Study" in the American Journal of Lifestyle Medicine on 27 March 2026. Coach-client working alliance was high. Goal attainment was positive. Validated measures of ADHD symptoms, executive functioning and functional impairment improved with medium to large effect sizes. The authors state the limits plainly: the design is single-arm with no control group, the coaches were credentialed, and the client sample was fairly homogenous in gender, race and education, so generalizing is constrained.
The most cited older result is Field, Parker, Sawilowsky and Rolands (2013), which studied Edge Foundation coaching for college students and found moderate to large effects on learning strategies and self-regulation, with 88 coached students against 39 comparisons.
What is missing is randomized controlled trials. Sibley's group said so in the same 2026 survey paper and treated the absence as the reason the field is not ready for licensure.
Our position: the evidence supports coaching as a skills intervention for people already stable enough to use a plan. It does not support coaching as a treatment for ADHD, and we will not describe it as one. We will not put a timeline on your results either, because we have not published the measurement that would justify one. Ask us on the call what we track and how, and hold every practice you speak to the same standard. If You Need an Accommodation Letter Buy the Assessment First
A coach's letter is usually not enough to secure a 504 plan, an IEP, college disability services, or a workplace accommodation under the ADA. Those processes want a diagnostic report from a qualified assessor, with testing behind it.
The order that works:
Assessment. A psychologist or neuropsychologist produces the report. This is the step people skip, and it is the one everything else depends on.
The accommodation request. School, university disability office, or employer HR, using that report.
Coaching. After the accommodations exist, because coaching is how someone learns to actually use extended time rather than waste it.
We can tell you what a report generally needs to contain for a school to act on it. We cannot write one. If a coach offers to, that is a reason to leave.
What Coaching Looks Like at The Ladder Method
Coaching starts with a consultation to assess needs and goals. We then match the client with the coach best suited to their personality and objectives. Sessions run weekly or biweekly and focus on skill development, accountability, and application to real situations. We hold regular progress check-ins with students, parents, or adult clients.
Graduation happens when the client demonstrates consistent independence and mastery of the strategies introduced during coaching. There is no renewal you have to decline.

Questions We Get Asked
Are you licensed?
No, and neither is most of this field. California does not license or regulate coaching and the title is unrestricted. Diagnosing or treating a mental health condition requires a licence from the Board of Behavioral Sciences or the Board of Psychology. We hold no clinical licence, we do not diagnose, and we do not treat ADHD. Ask every practice you speak to the same question.
What does ADHD coaching cost?
We quote our rate on the consultation call, before you commit to anything. Ask for it in the first 5 minutes.
Will insurance pay for ADHD coaching?
Almost never. The 2026 JAMA Network Open survey found fewer than 5% of ADHD coaches accept health insurance. California's SB 855 requires commercial plans to cover medically necessary treatment for mental health conditions including ADHD, but that applies to treatment by a licensed clinician rather than to coaching.
Can I use my HSA or FSA?
Sometimes, and less often than coaching sites imply. IRS guidance states that medical expenses must be primarily to alleviate or prevent a disability or illness and do not include expenses merely beneficial to general health. You need a diagnosed condition and a service treating it, and your plan administrator makes the call. Confirm in writing before you spend.
Is ADHD coaching the same as executive function coaching?
The work overlaps heavily. ADHD coaching assumes a diagnosis or a strong suspicion of one and builds around how that brain behaves. Our executive function coaching in West Hollywood page covers the same skill set without an ADHD frame.
Is it the same as therapy?
No, and the difference is legal rather than stylistic. Therapy in California requires a licence from the Board of Behavioral Sciences or the Board of Psychology. Coaching requires nothing. We build skills and accountability. We refer out anxiety, depression and trauma.
Is it the same as tutoring?
No. A tutor teaches the subject. We teach the system that gets work planned, started, finished and submitted.
Do I need a diagnosis to start?
No. Coaching does not require one. Most routes to reducing what you pay out of pocket do require documented eligibility, so you can start without a diagnosis and probably cannot get help paying for it without one.
Can you write a letter for my child's school or my employer?
Usually not on its own. Schools, universities and employers generally want a diagnostic report from a qualified assessor. We can tell you what such a report needs to contain. We cannot produce one.
How long before we see something?

How often are sessions?
Weekly or biweekly, set at the consultation based on goals and need.
What ages do you work with?
Elementary through college students, plus adults.
What if I already have an ADHD diagnosis and a therapist?
That is the client coaching works best for, because the clinical work and the systems work point at different things. Tell us who your therapist is and what they are working on so we do not duplicate or contradict it.
